While scheduling, we would also be happy to answer any additional questions you may have regarding your treatment or insurance benefits. How far in advance should I schedule an appointment?
We tend to book out 2-3 weeks in advance, so we recommend scheduling your appointment well ahead of time. If you need to be seen sooner but we don't have available appointments, please request to be placed on our waitlist. We will contact you when we receive cancellations that could work with your schedule. Do you have a waitlist?
Yes, we do have a waitlist! If you are unable to schedule an appointment at your preferred time or with your preferred therapist, we can put you on a list to be contacted whenever there are cancellations. Whenever there is an opening, we will either call, text, or email you to inform you of the opportunity. If we missed you, we will leave a voicemail with the available appointment date, time and therapist. Please keep in mind that we will be calling other patients until the spot has been filled. How frequently should I schedule physical therapy appointments?
During your initial evaluation, your therapist will work with you to create a custom treatment plan that details how frequently you should come in for an appointment. However, since we tend to book out several weeks in advance, we recommend scheduling a few follow-up appointments one or two weeks after your initial evaluation. If you end up not needing those follow-up appointments, you can always call us 24 hours in advance to cancel them. What is your cancellation policy?
We request that you cancel your appointment at least 24 hours in advance. Missed appointments or those canceled with little notice may be subject to a $50 fee. |
Your First AppointmentWhat should I bring to my first appointment?
Please bring photo ID, insurance card, and any doctors referral that you may have. We suggest that you bring or wear loose comfortable clothing and exercise appropriate shoe wear. We also have gowns and shorts if needed. How early should I arrive to my first appointment?
In preparation for your first appointment, we require our patients to complete our electronic forms respective to their treated area(s) and insurance plans. These forms can be completed either on your own personal device at home or in our clinic using our iPad. If we have your email on file, please expect an email to be sent about 1 business day prior to your scheduled appointment. If you have not completed or received your forms, please plan to arrive 15-20 minutes before your scheduled appointment to complete them. What can I expect from my initial evaluation?
A physical therapist will evaluate your condition and with your active participation, they will formulate a treatment plan to meet your goals. Treatment may include manual therapy techniques, modalities, and home management strategies. The session will last approximately 1 hour. Do you have accessible parking?
Parking is located directly in front of the facility. There are no stairs to negotiate and handicap parking is available. We also have bicycle parking rack for those commuting by bike. |

We understand that insurance can be confusing, so if you have any questions regarding your plan please give us a call. Below are just some of the insurances we accept.
What are my options if my insurance is not accepted by your clinic?
We can check with your insurance if you have any "out-of-network" benefits. Please note that any service charges for "out-of-network" insurances will be higher than it would be if charged in-network. If your policy does not include out-of-network benefits, the second option would be self-pay. What options do I have if I do not have insurance?
For those without insurance, we offer a self-pay option. We charge $140 for an initial evaluation and $120 for each subsequence follow up appointment. How much do appointments cost out-of-pocket?
