What should I expect for my first appointment?
In preparation for your first appointment, we require our patients to complete our electronic forms respective to their treated area(s) and insurance plans. These forms can be completed either on your own personal device at home or in our clinic using our iPad. If we have your email on file, please expect an email to be sent about 1 business day prior to your scheduled appointment. If you have not completed or have not been sent your forms, please plan to arrive 15-20 minutes before your scheduled appointment to do so. A physical therapist will evaluate your condition and with your active participation, he or she will formulate a treatment plan to meet your goals. Treatment may include manual therapy techniques, modalities, and home management strategies. The session will last approximately 1 hour.

What do I need to bring?
Please bring photo ID, insurance card, and any doctors referral that you may have. We suggest that you bring or wear loose comfortable clothing and exercise appropriate shoe wear. We also have gowns and shorts if needed.

What are manual therapy techniques?
Manual therapy includes many different types of techniques used to increase mobility and improve the function of joints, muscle, and connective tissue. Manual therapy is a hands-on treatment that may include procedures such as joint mobilization, cross-friction massage, muscle energy, myofascial release and contract-relax stretching.

Do you have accessible parking?
Parking is located directly in front of the facility. There are no stairs to negotiate and handicap parking is available. We also have bicycle parking rack for those commuting by bike.

What options do I have if I do not have insurance?
For those without insurance, we offer a self-pay option. We charge $140 for an initial evaluation and $120 for each subsequence follow up appointment.

What are my options if my insurance is not accepted by your clinic?
We can check with your insurance if you have any "out-of-network" benefits. Please note that any service charges for "out-of-network" insurances will be higher than it would be if charged in-network. If your policy does not include out-of-network benefits, the second option would be self-pay (see above).
